The 980 nm laser is particularly suited to this application, as it has high absorption in water and oxy hemoglobin, causing rapid vaporization of the bone and mucosal tissues, while ensuring that there is no bleeding. It has many times the water absorption of 810 nm wavelength, as well as more absorption in oxy hemoglobin than the 1064 nm wavelength. This can clearly be seen in the attached chart.
   

Surgical Technique

   
 
Instead of approaching the naso-lacrimal duct system by exposing it externally, the transcannicular approach involves introducing a special atruamatic cannula into the nasolacrimal duct from the lower punctum of the eye. A thin laser fiber is introduced into the cannula, and the whole procedure can be viewed using a nasal endoscope. With the laser fiber in contact with the bone, the laser energy is delivered to cause a vaporization of the bone, and entry of the laser fiber into the nose. This allows the stenosis of the naso-lacrimal duct to be obviated.

This useful procedure, to treat glaucoma refractory to more conventional treatments, is done with the special Star Lasers Cyclopexy probe. This special fiber optic probe with a ball shaped tip, allows diffuse delivery of the laser energy, which passes through the sclera, but which is absorbed in the melanin of the ciliary bodies.

Retinopexy
Transcleral diode laser retinopexy provides a convenient alternative to cryopexy during retinal reattachment surgery. It combines the closure of retinal breaks with an increase in retinal adhesion during retinal reattachment surgery. Cryopexy has the potential to stimulate PVR, and can also sometimes lead to cystoid macular edema. Diode Laser Retinopexy offers a safer alternative to cryopexy, using the special retinopexy probe developed by Star Lasers. A special optical arrangement at the fiber tip gives energy at right angles to the probe tip, which is very convenient. The distal end of the fiber is covered with a curved glass tube, which makes for easy scleral indentation.  The retinopexy probe can also be used for intra ocular tumors, ROP and neo vascular glaucoma. The 810 nm laser fiber does not stick to the sclera, and can easily penetrate through silicone exoplants.

The StarLas 30 provides a very convenient option for endophotocoagulation. The infrared wavelength is particularly suited for vitreo-retinal surgery, as it penetrates through opaque and cloudy media more effectively than a green light laser. The StarLas 30 provides a variety of options for endophotocoagulation, including straight, tapered, curved and 25 gauge handpieces.
